  Process Hazards Analysis (PHA)

Process Hazard Analysis (PHA) refers to a series of techniques for systematically reviewing a process to identify potential hazards and operability problems. OSHA Process Safety Management (29 C.F.R. § 1910.119(e)) and EPA Accidental Release Prevention Requirements for Program 3 (40 C.F.R. § 68.67) require a hazard analysis for all subject processes. Acceptable PHA methodologies include:

 
  • What-If?/Checklist,
  • Hazard and Operability Study (HAZOP),
  • Failure Modes and Events Analysis (FMEA), and
  • Fault Tree Analysis (FTA)
